Durable Power of Attorney

A power of attorney gives a designated person the legal authority to act
on one's behalf.  A durable power of attorney designates a person as
your "attorney-in-fact" and remains in effect or becomes effective when
you become incapacitated or mentally incompetent.

Health Care Power of Attorney

Generally, a person has the right to control their own health care
decisions.  A health care power of attorney allows a person to designate
someone to make  decisions for them in the event they become
incapacitated or incompetent.  This person then acts as a health care
agent and makes health care decisions on their behalf.
